Malar Surgery
What is Malar Surgery?
Malar surgery (cheek augmentation or reduction) is a surgical procedure to reshape or reposition the cheekbones (malar bones), enhancing facial harmony, definition, and youthfulness.
Cheekbones are key to facial aesthetics, providing volume, contour, and balance. When underprojected or disproportionate, they can create a flat or aged appearance. Malar surgery allows customization—either enhancing or reducing cheek prominence—based on patient goals.
The chin also plays a vital role in facial proportion, especially in profile. A receding or overly prominent chin can disrupt balance with the nose or neck. Mentoplasty (chin surgery) corrects these asymmetries, improving both aesthetics and, in some cases, function.

Recovery Time
- First few days: Facial swelling, tightness, or temporary numbness.
- Week 1: Soft diet recommended if intraoral incisions were used.
- Week 2+: Gradual return to daily activities.
- 3 months: Final results (full tissue adaptation).

How is the procedure performed?
The technique varies depending on whether the goal is to increase or reduce malar volume:
Cheek Augmentation (Augmentation Malarplasty)
This can be done by placing solid, biocompatible silicone implants or through fat transfer, using the patient’s own fat.
The implants are usually inserted through an intraoral incision (inside the mouth), which avoids visible scars.
This technique allows for a controlled and natural projection of the cheekbones.
Cheek Reduction (Reduction Malarplasty)
This is done by shaving or reshaping the malar bone in patients with excessively prominent or asymmetric cheekbones.
The incision is also usually intraoral, allowing the procedure to be performed without visible marks.
Procedure duration: between 1 and 2 hours.
Type of anesthesia: local with sedation or general, depending on the case.
